A Wellness Profile will allow you to learn more about your client and what they do in their day to day lives. This will help you customize a plan, make product recommendations and guide them to success on their wellness journey.

Wellness Evaluation via Email or Text Message:
It is always best to try to do the Wellness Evaluation in-person or over the phone (or zoom). However, we will occasionally come across some people that are just too busy and you need to do the wellness evaluation via email/ text message. Here are a list of questions that you can send to them which will gather most of the information you need in order to personalize a plan for them.
1. What are your Wellness Goals?
2. Current Weight?
3. Goal Weight? By a Certain time/event?
4. Height?
5. What time do you wake up?
6. What time is breakfast and what do you have?
7. What time is lunch and what do yo have?
8. What time is dinner and what do you have?
9. Snacks in-between? If so, what do you have?
10. Rate your energy on a scale from 1 – 10 (10 being the best).
11. How many ounces of water do you drink on a daily basis?
12. How many times a week do you eat out? Where? Average cost of meal.
